Have you tried using the "snapshot" tool in Adobe Reader 6.0 to save the information 
(as a screen image) in the fields.?
If that works, PDF-SnapShot  http://www.pdfstore.com/details.asp?ProdID=680
is an inexpensive tool   to automate taking "snapshots" and automatically converting 
the snapshot
into a PDF file - without the need for the full version of Acrobat.
