File attachments work in Reader 5.1 and newer if the document has Extended File and Comments rights.
Of course, in order to get the Right to assign Rights, you would have to procure the Adobe Document Server for Reader Extensions. For a quote and pricing information, you best contact the Adobe _Enterprise_ Sales representative for your business type in your country.
Hope, this can help.

In Acrobat you can attach a file to the form using the File Attachment Tool. Only people using Reader or Approval don’t get it, only those will full Acrobat. Does anyone know a way around this? I have Adobe forms for “canned” motions but they need Declarations which can’t be done in Adobe since we don’t know how much room they would need to state the facts. So I want to link them to a Word Template from the motion. When I found the File Attach tool I thought it was great but when I turned the form over to my users, the paperclip icon did not work. Any ideas? Thanks.
