As best I can understand what you are after, it is done for you by the tools you use 
to make 
the PDF. Adobe's PDFMaker runs on Word files, sees what is there, and puts the 
clickability 
in to the PDF. As a user, all you have to say is what you want included. So TOCs, 
hyperlinks, will all get carried over.

And just a note on terminology: the equivalent sort of thing (not quite the same 
functios) to a 
bookmark in PDF is a 'named destination'. A 'bookmark' in PDF is an entry in the 
bookmarks 
palette that gives an hierarchical overview of the document.
