We often distribute on CD pdfs that I usually break down into say chapters, keeping the files less than 5 MB or so. If they weren't broken up like this the file maybe from 50 - 100 MB in size.
OK...
Have you looked at PDF reduction tools such as PDF Enhancer (http://www.pdfsages.com/enhancer.html)?
How does Acrobat Reader handle files like this?
Just fine. I've seen Reader handle files MUCH larger than that...
How much of a file is loaded into memory when you open it?
Only what it necessary to display page 1 (plus some structural information for later operation).
When you open say the 51st page and then start scrolling up or down? How are large files handled on machines with limited RAM?
Depends on the OS, version of Acrobat, etc.
Large files on the Web is a different but interesting thing too. I understand that if the server is configured correctly and the file has been optimised it will be byte served so that only the requested pages are downloaded.
Not optimized - "Fast Web View" Enabled.
