One of these forms came from Adobe (the ACE information collection form),
one from the IRS (tax form). I'm not sure how they were created - don't have
them around any more. I've stopped using Macintosh Acrobat 6 for forms until
I figure this out.
My attempt to fill out the forms was completely in Acrobat 6 Professional.
