Jon sent me this reply and option #1 worked:
You have either an RGB or LAB colour space in your PDF - nothing to do with colour settings.
You have a couple of options
1) In the EPS save dialogue - allow the RGB/LAB colour space
2) Pre-flight the PDF early in the workflow and bounce-back the PDF to the originator
3) Edit the rogue image in Photoshop - CONTROL-CLICK the image and EDIT
4) Change the rogue image in a plug-in such as Enfocus Pitstop
