This is something which goes far beyond the capability of Acrobat and a plug-in.
For such a project, it is HIGHLY recommended to start working with a dedicated forms editor.
You might have a closer look at the following editors:
• OneForm Plus by Amgraf (http://www.amgraf.com)
• LiquidForms by Cardiff (http://www.cardiff.com)
• Adobe Forms Designer (note that the current version is not really recommended, but they are about to start a public beta of their new thingie, which is said to be pretty usable ... but I haven't seen it yet, and there is no release date given).
Note that your customer should not fall into the trap of scanning existing paper forms and simply slapping fields on it, because that would definitely create bloated or badly printing forms.
Another thing to make your customer think about would be to really take that opportunity and to consider a complete evaluation of their forms collection. This step (going to an electronic version (meaning PDF, otherwise, you would not have asked here, wouldn't you)) is a chance of a lifetime.

Can anybody recommend a forms creation and editing plug in for Acrobat. A
customer of ours is planning archive thousand of forms to electronic format
and also create new forms. Also a way to make a scan to electronic form
available for electronic data entry?
