IIRC, there are two different ways to accomplish this:

1) Set an "On Open" script in the FDF that populates all your fields.

2) Populate the fields directly in the FDF.


"2" is quite a bit more straight-forward, but "1" is handy to know about
too.

Oh, you want syntax?  Some people... ;)

Display* is what the users see, Value* is the value actualy submitted.

The JavaScript dictionary can contain up to 3 different scripts that are
executed at various times.  I'm not entirely sure what those times are, but
a little experimentation on your part should get you an answer in no time.
