Okay, if this is the case then have you looked at the PDBookmark object in the Core API spec? There are some methods in there that allow you to pinpoint exactly where you want to insert your bookmark (i.e. PDBookmarkAddChild, PDBookmarkAddNext, PDBookmarkAddNewSibling).
