Do you know if any one has developed any FDF toolkit extensions that might
deal with annotations, etc.

        Either building them yourself in the FDF OR you could add 
JavaScript to the FDF and the JS would create the Annots on loading...

        If you go the route of "native" FDF annots, I can't think of 
a way other than what you suggest.  If you go the JS route, those can 
be added via the FDFToolkit's JS calls.
