After upgrading to firmware v1.10, I let my *ist D format my 512MB CF
card. I checked the CF card on my WinXP machine, and it was reported as
FAT. So that is FAT16.
I then did a format of the CF card with my CF, and changed it to FAT32.
Then I put it back in the *ist D. The camera could read it without any
problem, and write a PEF file to it.

Earlier it was stated operating systems don't support FAT16 over 2GB
size due to too big cluster sizes. So I would conclude from this, that
the *ist D can use >2GB cards if you format them first on a PC using
FAT32.

Is this correct?
