I have not experienced it, but I could see it happening with a worn 
drive mode lock, or simple lack of care when switching drive modes, 
Self-timer is right next to Cs (Continuous Silent), the most useful 
drive mode.

I suspect most cases of this are from simple lack of paying attention.
