I've been guilty of this on occasion.  When I'm in a dirty environment,
(so I keep the lens cap on except when taking the shot) or if I'm just
grabbing one shot quickly before setting up properly, or if I'm using
a polariser on an old lens, having the lens hood mounted can be a bit
of a nuisance.  Sure, I could take the hood off and toss it into the
camera bag, but it's just as easy to leave it in the storage position.

That little cutout on the DA* hoods is one of my favourite features.



As to the original image; it looks like pretty soft light, and I'd
guess the light is slightly behind her and to her left, so there's
not much of a risk of lens flare spoiling her shot.
