I've got some generic lightstand mounts and 5/8 studs with 1/4-20 threads on them, so I bought a couple of these:

They're like the ones John posted, but for a dollar more you get a mail PC plug also.

Both Camera stores here in Raleigh carry generic Hot Shoe/Tripod Mount
adapters (with PC connector):

If you're looking for tilt/swivel adapters, look for metal at the
locking screws.

The problem with it is that when I unintentionally tightened the lower
mounting screw a little too tight and that plastic base split. Now it
won't tighten at all.

Better is something like this one, which is all metal:

Which is what I replaced the broken one with.
