That was the 28-70 f/4.0 not the 2.8.

I can vouch for that, I had an early 28-70 f4.0 that did just that. It appears that it was a composite molded plastic and glass aspherical element that failed to hold together. The second copy that I acquired, seems to be holding together, though it's construction is well plastic-ey. Sadly the plastic fantastic FA zooms don't seem to wear that well mechanically.
