Actually, it's easier than that. In your Protel root (mine is "c:\Program Files\Design Explorer 99SE"), go to the "System\Templates" directory and there will be a "Default.tdb".
Right click, "Open With..." Protel and you will notice that is a normal DDB - in fact it is the database template that is loaded when you start a new design. Create a new PCB in that and modify it to suit your needs and save it. Next time you start a new database, it will load that one. Maybe there is an easier way but I have just always used default files to start a project or sometimes I load a previous project and similarly run from there.
