Jencks says there's a review article by Judge et al, "Income 
Inequality and Population Health" in Social Science & Medicine, Vol 
46, pp 567-579, (1998), that trashes the Wilkinson thesis. As he puts 
it, Judge is pretty partisan, but so is Wilkinson.
