I have found out my answer from the perl-win32-database mailing.
try
ppm install http://www.cpan.org/authors/id/JWIED/DBD-mysql-1.2212.x86.ppd

Am I right in assuming you tried this with the latest version of perl, I had
the same and after a very frustrating 2 weeks went back to perl 5.15 and
installed it without problems. Should you find an alternative please let me
know. (I went back as I needed prductivity more that perl 5.6)

and when I do install I got the following errors:
ppm install dbd-mysql.ppd
Error installing package 'dbd-mysql.ppd': Could not locate a
