I've seen a lot of questions on setting up Perl
(ISAPI, .pl extensions, etc) on NT and IIS4 & 5. Even though the
newer versions of Active State do a lot of the installation for you - there are
still trouble spots you need to watch for. Take a look at the following

Hope this helps, I had a lot of the same problems
when I first started using perl with NT.
