eg

perlpmc_foo()
struct perlio_bar
typedef struct perlio_bar Perlio_bar
#define PERLPMC_readonly_TEST ...

ie essentially we are making the subsystem name more verbose. This kind of
matches the way things are currently done in PerlIO.
