For your "dpkg" error, try this from a command shell:
$sudo apt-get update --fix-missing
$sudo apt-get upgrade
$sudo dpkg --configure -a
I have seen the missing window icons problem in Ubuntu when Compiz is set to a
higher level than the hardware can tolerate. I suspect that you have set them
to have "extra" effects. Either knock it back a setting, or turn it off and
your icons should reappear. You will then need to trouble shoot what hardware
is giving Compiz problems.
