I recommend adding a libpqsrv_cancel() function to libpq-be-fe-helpers.h, to use from dblink and postgres_fdw. pgxn modules calling PQcancel() from the backend (citus pg_bulkload plproxy pmpp) then have a better chance to adopt the new way.
