I've looked at the patches and ISTM these work as expected.
IsRelationExtensionLockHeld and IsPageLockHeld are used only when
assertion is enabled. So how about making CheckAndSetLockHeld work
only if USE_ASSERT_CHECKING to avoid overheads?
