Remove this from the documentation, and:

  - Other minor cleanups (thanks KaiGai)
  - Added pg_dump support
  - Added support for 'ALL(col1)' grant/revokes (helped with pg_dump)
  - Added more regression tests
  - Updated documentation accordingly

Please test, comment, etc.
