PHP: 4.2.1 (Installed from zip archive)
IIS: 5.0
Win2K - Latest patches as of 1st June 2002.

I've also noticed this problem with 4.2.1 when using MS SQL functions.
The address field varies.

When setting "magic_quotes_runtime = Off" in php.ini the problem
dissapears. Seting this to on and the problem re-appears.

This is 100% reproducable on my system. YMMV.

I received the same problem.I am working with php version 4.2.1 , IIS
5.0, Windows 2000.
I configured my IIS server using php4isapi.dll.
After number of refreshes I receive:
"Php has encountered an Access Violation at 01010290E".
Since that I have to restart my IIS server in order
to continue a work.
I saw the similar problem with php , version 4.0.
I hoped that the upgrade to 4.2.1 will solve this problem,
but it doesn't.
This problem doesn't occure working with php.exe.

1. Make sure "Cache applications" is ticked. It's when PHP gets
unloaded that it has Access Violations.
2. Run iisreset.exe

I'm following this step
" Under 'Home Directory', click on the 'Configuration' button.
Add a new entry to the Application Mappings. Use the path
to the php4isapi.dll as the Executable, supply .php as the
extension, leave Method exclusions blank, and check the
Script engine checkbox."
and when I get "PHP has encountered an Access Violation at 0129E68A" in
my Web browser
