Are you sure you have downloaded the thread safe build and not the non thread safe version (nts)? That being said, I strongly recommend to use fastcgi instead of the ISAPI with IIS5/6/7. ISAPI is missing. After running it, IIS is not configured to run PHP, and PHP is not listed in any of the ISAPI extension handlers at all.
