Here's more information.

I have the same problem.  PHP works in CGI mode fine.  When used in
ISAPI mode it keeps giving me an Access Violation.  Scripts originally
work and then no matter what you do with PHP it throws an access
violation.  The file could be just <?php phpinfo(); ?> and it will
cause an access violation.  This is under IIS 6 on Windows XP Pro.

The CGI version is OK but the isapi gives me the folowing error

PHP has encountered an Access Violation at 0B292466

With any kind of code with or without db connection. (eg phpinfo() i a
page)
