This bug has been fixed in CVS.

In case this was a PHP problem, snapshots of the sources are packaged
every three hours; this change will be in the next snapshot. You can
grab the snapshot at http://snaps.php.net/.
 
In case this was a documentation problem, the fix will show up soon at
http://www.php.net/manual/.

In case this was a PHP.net website problem, the change will show
up on the PHP.net site and on the mirror sites in short time.

Got the same error (first four characters left off strings) using
another (not phpMyAdmin) script on:
FreeBSD with PHP (4.2.3) and MySQL (3.23.52)

Looks like a PHP/MySQL bug, not phpMyAdmin.

I couldn't recreate the bug with your examples.
It worked for me.
I am using Apache 2.x php 4.23 and mysql 3.23.53
Did you scroll across to make make a primary key.
The last option is not visible unless you scroll horizontally.
But it is probably something else on your system.
