Bug description:max_input_vars warning unknown file

Description:
------------
When PHP raises a warning about max_input_vars being exceeded, it would be
very helpful to see which file actually caused the error. The current error
message just says "Unknown on line 0".

Expected result:
----------------
Script file name

Actual result:
--------------
Unknown file
