Exactly the same problem.

Additional note:

Same effect without .htaccess but auto_prepend_file within VirtualHost
settings.

How to reproduce it?

Install Apache2 (no matter what configure string). Install PHP (as
module, not matter what configure string). Use the php-ini.dist and the
default httpd.conf. Set up two name based virtual hosts. One with
auto_prepend_file and one without. Sometimes the auto prepended file
will be auto prepended for the virtual host which don't have that
setting. A workaround is, to set 'php_value auto_prepend_file none' for
the second host. Please fix that bug!

Oh, my bad. We can't really do anything about this unless we can
reproduce this ourselves so you need to provide us all the necessary
information how to do it. So you need to create shortest possible
httpd.conf, php.ini, .htaccess, etc. which we can then use to setup our
own test environment.

Ah, responded to that in our 11 Jul exchange hence my confussion.

./configure --prefix=/usr/local/apache2 --enable-mods-shared=most
--enable-ssl

Do you want the whole configuration file?

Its a pretty standard configuration. Regular host and a couple of
virtual hosts.

Lines directly relevent to php:

LoadModule php4_module   modules/libphp4.so

DirectoryIndex index.html index.php index.shtml index.html.var

AddType application/x-httpd-php .php

ErrorDocument lines all pointing to /errors.php
