The IIS5 part is irrelevant, I think...
Dig around for a file++ whose name is composed of "GD", "PHP", and ".DLL".
Put it next to all your other PHP DLL files.
Alter your php.ini to refer to it the way it refers to other PHP DLLs.
It's Windows: Pray.
