If this is truly the code you're using, you're missing the closing 
curly-brace after the else statement. I.e. this:
} else {
      echo "<font size=+1>Your Information has successfully been 
entered into the database!</font><br>";

Should be this:
} else {
      echo "<font size=+1>Your Information has successfully been 
entered into the database!</font><br>";
       }
