Try to apply xsltproc to manual.xml. Uncomment all the other function parts if you want a quick build. I think if you start from that point, it will properly select the right template. I do use the htmlhelp.xsl to build up the new CHM manuals, but it produces mostly ready output for the HTML Help compiler, and is really not viewable without any skin (provided only in the CHM), so I would not like to advise you to try that one...
