[PHP-DOC] Parameters passed by reference in function definitions

Hello Goba and others!

It seems that <parameter role="reference"> works perfect with the old
stylesheets. Should be the old style with &amp; changed to this new
style all around the docs? However it seems that the old style works in
Livedocs already (at least at livedocs.phpp.org) so there is maybe no
need for this change.
