Bug description:  Incorrect indexes in CHM Manual 

Description:

The newest CHM manual (Sep 1, all languages) has a major problem.  The
index page on left pane is completely out of date and is not corresponding
with the actural table of contents.  This problem persist for days.

Another open bug #33838 states '?????' in English manual, which still
exists.
