Hello everyone,

There are several deprecated extensions that serve no purpose currently (like the service they depend on hasn't been around for years...) so let's do something about it.

The following extensions could be removed from the docs with no alternatives except adding aspell->pspell to the 404:

 aspell: deprecated forever, confusing
 ccvs: no longer relevant to the world
 cybercash: no longer relevant to the world
 cybermut: does anyone know if this still works? 404's all around
 mcal: unmaintained, no releases, doubt it still works
 muscat: unmaintained, no longer works
 pfpro: no longer relevant to the world
 satellite: deprecated, experimental, old, dead
 sesam: php 3 only

And there are a few more that may or may not stay, depending on if we feel enough time has passed. These are:

 clibpdf: deprecated, people are told not to use, but people still do
 dbm: dead, may be safe to remove
ircg: the source was manually removed from php.net servers last year, should keep for awhile
 oracle: worth keeping, but we can promote oci8 a little better here

The proposed path:

A) Delete sources from phpdoc-all/*/
B) Create appendix of removed extensions, including all ids

One note: if people _really_ need these docs I'm sure they will find them as there are a zillion old PHP manuals online and archive.org. I sure hope people using the above have already rewritten their code with alternatives though...
