With the kind help of cweiske on IRC I realized pear install was
broken, i have nuked them one by one with pear uninstall -n
phpdocs/... and now

php ../phd/render.php -d doc-base/.manual.xml -o output_dir/ -f php -P PHP

works. Great :)
