Details:
I know PhpGW isn't supporting yet php5 but this would be a nice step
forward.
The page is displayed blank because class.uiaddressbook.inc.php declares
$template twice. It was a mere warning in php4 but it's not a fatal one in
php5.
Removing the second declaration fixes the trouble
