Which does show some php5 files as well as php4, not sure if the mixture is 
causing the problem.

the pipe | is an "or", so "php5-pgsql | php5-mysql | php4-pgsql |
php4-mysql" means "if php5-pgsql or php5-mysql or php4-pgsql or
php4-mysql is installed then dependency is met".

for the grant, if you issue as root and it doesn't save the grant then
I don't know what's wrong, but that would be a mysql issue.
alternatively, you can delete the phpgw database and user from mysql
and in step1 give it the root user and password it will create the
user and database that you entered in the header manager.  The howto
linked form the downloads page explains in more detail.
