1)  If the application is writing to the file, it should be in /var/opt, 
not in /opt.

2)  This sounds like a problem that I encountered.  I never got a recent 
version of pkg to test against, so never pursued it or filed a bug.  My 
analysis indicated that the problem was that pkg would attempt to rename 
the file (or directory) over into its lost+found directory, which would 
fail if the file and the lost+found directory were on different file 
systems.
