The following webrev contains fixes for the following issues:
8613 image-update does not cross publisher boundaries despite
preferred status
8616 operations should use same publisher when only non-preferred
has package

Note that this webrev is dependent on the changes made in the webrev for
bug 8487; it will be putback after.
Change Summary:
* fixed image-update to respect preferred publisher status
* fixed install operations to use same publisher for dependencies if
dependency is not offered by preferred publisher, but multiple
non-preferred have it
* fixed package state code to build fmri with correct publisher prefix
by stripping preferred publisher prefix from publishers that are no
longer preferred from installed packages
* added several unit tests to verify all of the above
