The following webrev contains fixes for the following issues;
13021 file manager exceptions should be api exceptions
12265 pkg publisher -a can trigger permissions traceback for image
needing upgrade
13877 pkg info -r broken for unprivileged users when manifest
retrieval required

This set of changes in general should greatly improve the robustness of
the client in the face of permissions errors. It also adds several unit
tests to ensure that remains the case in the future.
Please note that this changeset intentionally does not address bug 6353,
although it should make it (indirectly) easier to do so.
