Bug:
18464 revoka-ca-cert needs a rethink
This webrev changes what we hash to keep track of the identities of
certificates.
This change will constitute a flag day for anyone who has made use of
the approve-ca-cert or revoke-ca-cert options to set-publisher. Any
users will need to unset their existing certs/hashes and reapprove or
rerevoke the certificates.
