Interesting read. The only suggestion I can provide to you is that the initrd somehow must launch two inits. I have no idea how it can happen, but it seem unlikely that the kernel starts to initrds, and there is nothing I am aware outside the initrd that start init. Good to hear that it is solved, even though you do not know how it was solved.
