Which frontend are you using: nm-applet (GNOME) or the network-manager-kde resp. the plasma widget? What are the contents of /var/lib/NetworkManager/NetworkManager.state? Does it help if you stop NetworkManager, remove the file (after making a backup of it), and starting NM again?
