httpd.${HTTPD_MPM}
if this exceeds 8bytes, the process name is truncated in /proc/$pid/status 
(/proc/$pid/cmdline?) and pidof program fails to locate pid.

i tested on my system, and did not observe such situation right now. but i'm 
pretty sure it did exist on other older systems.
kernel 2.6.9. 

so perhaps you could execute:
$ doexec cat DAAAAAAAAAAAAAAAAAAAAAAAAAA
and see if pidof "sees" pid of that cat program:

$ pidof DAAAAAAAAAAAAAAAAAAAAAAAAAA
846
