Let's go for the low-hanging fruit first and wait and see on the rest. Thus, I feel the priorities should be implementing correct transparent background for cairo file devices, then see what is possible with xcairo in this regard, then at that point if necessary (after all, transparent backgrounds might not be possible at all with xcairo) decide on the issue you are concerned about.
