I've had that problem repeatedly on the Sony branch -- documents
that are not in default will get marked as default plus whatever
category they're in (though I have multiple select turned off),
and documents I delete undelete themselves and sometimes show up
twice. It's sort of a chore to manage documents with plucker-sony,
though the configurable jogdial support is great.
