That is going to be very difficult. If your main office has to be able
to call your regional offices, you'd need to have your regional offices
connect to your registrar (if using SIP, a registrar is something like a
server for voip services).
Probably, you'd text your regional office to connect, before calling
them. That is if there is no other way for them to be connected to the
internet all the time. Or your internal WAN if you have one.
Point is, you'd need to have all your offices connected (be it internet
or WAN) to use VoIP effectively.
If you have the regional offices connected, then you'd probably setup
asterisk in your main office. The rest of the regional offices connect
to that one. Then later, you'll have a cluster, where each regional
offices have their own asterisk and have the other offices as peers.
There's many ways to skin a cat, and you'd probably figure out one that
will work for your organization. But imho, you want to start with one
asterisk first.
