did you use raidhotadd ? I had this before, its either raidhotadd
or raidhotinsert yata...(cant remember which).    You have to reinsert
the disk again into the array. 

watch -n 2 cat /proc/mdstat

 ..watch it reconstruct (its fun).

never read the manual but the howto's on raid are fine.
